Community Care Packages
Community Care is funded by the Commonwealth Government. They are planned and coordinated Care Packages assisting aged people to remain living in their own homes, rather than moving to Residential Care facility.
What Help is Available?
Services include help with:
- bathing, showering, or personal hygiene;
- social support;
- transport;
- laundry;
- meal preparation,
- assistance with medications
- cleaning
- shopping
It is important to know that the services provided can change, as care needs change.
How Can You Apply?
To be eligible you must first have an assessment completed by the Aged Care Assessment Team (ACAT) which can be organised through your local doctor or through the hospital. If it is deemed a Community Aged Care Package is appropriate for your needs, you will be referred to the RSL Home Care Service Coordinator.
How Much Does It Cost?
In most cases, you will be charged a fee for your Community Aged Care Package. The fees are subsidised by the Australian Government. If you are on the basic pension fees cannot be more than 17.5% of your pension. If you are on a higher income you may be asked to pay an additional fee of no more than 50% of any income above the maximum pension rate (these details are current as of February 2005, for clarification please contact the Aged and Community Care Information Line on 1800 500 853).
How Is The Package Arranged?
If the RSL Home Care Service is able to allocated a package to you, a package of services will be tailored to meet your needs as assessed by the Aged Care Assessment Team. You will then discuss with our Home Care Coordinator the options available for meeting your care needs and a plan of care will be agreed upon. After you have agreed on your care, a copy of your Care Plan will be given to you setting out the services you will receive.
How Is It Monitored?
The Australian Government has specific Community Care Standards, which providers such as RSL Care WA are legally required to meet, ensuring care recipients receive care of the highest quality. All those who are eligible for or receive services from a Community Aged Care Package provider:
- are entitled to quality services that meet their assessed needs;
- are entitled, where possible, to their preferred level of social independence; and
- should have their dignity and privacy respected at all times.
